Hazelwood New Vista

At Hazelwood, our mission is to assist individuals in developing skills and interests to reach personal goals and live meaningful and balanced lives. Hazelwood is an Intermediate Care Facility (ICF) providing residential services for individuals with intellectual and developmental disabilities (IDD). Hazelwood is located on 52 acres in Louisville, Kentucky and opened in 1972. In addition to the main facility, Hazelwood also includes three community ICF homes; Del Maria (located in Fern Creek), Windsong and Meadows. New Vista began management of Hazelwood in 2018. Staff provide care 24 hours a day, 7 days a week ensuring services, created by an interdisciplinary team, are fulfilled for each resident.
